You Can’t Outsupplement Your Foundations
Why Extraordinary Health Is Built Between Appointments
If you’ve been following along with this month’s series, you may be expecting me to finally tell you which supplements I recommend for supporting the immune system.
We’re almost there.
But before we talk about what we can add, we have to talk about what the body needs before any treatment has the opportunity to work well.
Because one of the most important lessons I’ve learned as a doctor is this:
You cannot build extraordinary health on an unstable foundation.
Patients ask me every week what the best supplement is for supporting the immune system.
My answer usually surprises them.
The best supplement in the world cannot consistently overcome four hours of sleep, chronically elevated blood sugar, a diet that doesn’t provide enough protein to rebuild tissue, or stress that has been running uninterrupted for years.
That doesn’t mean supplements don’t work. I recommend them every single day, and I have seen them make remarkable differences in the right clinical situations.
But supplements are exactly what their name suggests.
They are meant to supplement healthy physiology, not replace the foundations that healthy physiology depends upon.
It is a little like trying to build a beautiful home on unstable soil. You can invest in premium flooring, designer lighting, and custom cabinetry, but if the foundation beneath the house continues to shift, those improvements can only accomplish so much. Eventually, cracks begin to appear, not because the finishes were poor, but because the structure underneath never had the stability it needed.
The human body is remarkably similar. Every day it is attempting to regulate blood sugar, repair damaged tissues, produce hormones, coordinate immune responses, detoxify environmental exposures, digest food, adapt to stress, maintain muscle, and constantly restore balance. Every one of those processes depends on raw materials, adequate energy, and opportunities for recovery.
The body cannot build health from materials it never receives. That sentence has become increasingly important to me throughout my years in practice because I think it explains why some people make dramatic progress while others continue feeling stuck despite trying treatment after treatment.
The difference often isn’t motivation. It isn’t intelligence. It certainly isn’t willpower. More often than not, it comes back to whether the foundations of health have become strong enough to support everything else we’re asking the body to do.
We Live in a World That Prioritizes Intervention Over Foundation
Modern healthcare has become remarkably good at developing interventions. We have medications that save lives every day. Surgical techniques continue to advance. Functional testing has become increasingly sophisticated. Nutritional science continues to evolve, and there are countless evidence-based supplements that can play an important role in patient care.
I am grateful for every one of those tools. I use many of them myself. But I also think our healthcare system has unintentionally conditioned us to believe that the next intervention is always the answer. If we aren’t sleeping well, we look for a supplement. If our energy is low, we look for another supplement. If inflammation persists, we search for another laboratory test or another protocol.
Sometimes those next steps are absolutely appropriate. Other times, however, we are asking highly specialized treatments to compensate for physiology that never had the basic ingredients it needed to function well in the first place.
Imagine asking a professional contractor to build a home without lumber, concrete, or nails. No matter how skilled that contractor may be, there is only so much they can accomplish without the necessary materials. Our cells operate under the same limitations.
Every immune cell requires amino acids to build proteins, fatty acids to construct healthy cell membranes, vitamins and minerals to serve as enzyme cofactors, antioxidants to manage oxidative stress, and energy to carry out the thousands of biochemical reactions required for normal function.¹
The body cannot manufacture those resources out of thin air. It depends on us to provide them. That realization fundamentally changed the way I practice medicine. Instead of asking only, “What treatment does this patient need?” I also find myself asking, “Does this body currently have the resources to respond well to treatment?”
Sometimes the answer is yes. Sometimes the answer is not yet. That distinction matters far more than many people realize.
Healing Is a Partnership
One of the hardest conversations I occasionally have in clinic isn’t about a difficult diagnosis or a concerning laboratory result.
It’s with a patient who feels understandably discouraged because they aren’t improving as quickly as they had hoped. As we begin reviewing their treatment plan together, I sometimes discover that very little of it has actually been implemented. Not because they don’t care. Not because they aren’t trying. Life simply became busy.
Work became overwhelming. Children needed attention. A family member became ill. Travel interrupted routines. Healthy habits slowly slipped down the priority list until weeks or months had passed. I think every one of us can relate to that. Life has a remarkable way of competing with our best intentions.
Those conversations are never about blame. They’re about recognizing one of the most important truths in medicine.
No treatment plan works until it becomes something you actually do.
As doctors, we spend years learning physiology, interpreting laboratory data, evaluating research, and developing individualized treatment plans. We consider medication interactions, nutrient deficiencies, environmental exposures, hormone patterns, genetics, microbiome health, and countless other variables because we genuinely want to help our patients heal.
I love that part of my job. And I love sharing it with you, my patients. But eventually every treatment plan reaches the same place. It leaves my office. That is where your work begins.
I can’t prepare your breakfast. I can’t strength train for you. I can’t turn off your phone an hour before bed. I can’t practice stress management on your behalf. I can’t remember to take your supplements every morning. I can’t choose protein instead of skipping lunch. At some point, healing becomes less about creating another treatment plan and more about consistently living the one we already have.
I don’t say that to make anyone feel guilty. I say it because I think it is one of the most empowering realities in medicine. If your daily decisions influence your physiology, then they also represent some of the greatest opportunities you have to change it. That means you are not simply waiting for your body to heal. You are actively participating in the process.
Small Decisions Become Extraordinary Biology
One of the reasons I enjoy practicing naturopathic medicine is that it constantly reminds me how interconnected the human body truly is. Patients often think of sleep, nutrition, exercise, stress, digestion, and immune function as separate topics. Physiology does not.
Every night of restorative sleep changes hormone regulation the following day. Every meal influences blood sugar, inflammatory signaling, and the nutrients available for tissue repair. Every resistance training session stimulates muscle proteins, improves insulin sensitivity, and changes how the immune system communicates with the rest of the body. Every period of chronic stress alters nervous system activity, hormone production, and immune regulation.
The body experiences these events as one continuous conversation. Over time, those conversations become our biology. That is why I often tell patients that health is rarely determined by one spectacular decision. It is shaped by hundreds of ordinary ones. The walk after dinner. The extra serving of protein. The decision to go to bed thirty minutes earlier. The strength-training session that almost didn’t happen. The afternoon spent outside instead of in front of another screen.
Individually, none of those choices feels life changing. Collectively, they become the environment in which every cell in the body must live. And when we consistently create an environment that supports healthy physiology, something remarkable begins to happen. The treatments we prescribe often start working better because the body finally has the resources it needed all along.
Physiological Reserve Is Your Greatest Health Insurance
One of the concepts I wish more people understood is something called physiological reserve. Although it isn’t a phrase that comes up often outside of medicine, it helps explain why two people can experience the exact same stressor and respond completely differently.
Imagine two people board the same airplane. Both are exposed to the same recycled air, the same long travel day, the same disruption in sleep, the same time zone changes, and perhaps even the same respiratory virus sitting a few rows away. One returns home feeling completely fine. The other spends the next week sick in bed.
Or consider two parents whose children bring home the inevitable back-to-school cold. One parent develops mild congestion for a day or two and quickly recovers. The other develops a lingering illness that seems to drag on for weeks. Those differences are rarely explained by one vitamin deficiency or one supplement. More often, they reflect the amount of physiological reserve each person’s body had available before the challenge ever occurred.
I like to compare physiological reserve to a savings account. Every night of restorative sleep is a deposit. Every balanced meal is a deposit. Every strength training session. Every walk outside. Every opportunity to recover after a stressful week. Every time you choose to nourish your body instead of simply getting through another busy day. Those deposits accumulate quietly over time.
Then life happens. A demanding project at work. A new baby. A family emergency. International travel. A viral infection. A surgery. An unexpected diagnosis. Those are withdrawals.
The goal isn’t to avoid withdrawals. They’re part of being human. The goal is to have enough reserve that the withdrawal doesn’t leave your account empty. This is one of the reasons I often encourage patients to stop thinking about health as something they work on only when they don’t feel well. The healthiest physiology is built long before it’s needed.
Sleep Is When Recovery Becomes Biology
If I had to identify the single habit that influences almost every system in the body, sleep would be at the top of the list. We often think of sleep as passive because we are lying still with our eyes closed. Physiologically, however, sleep is one of the busiest periods of the entire day.
While we sleep, the brain clears metabolic waste products through the glymphatic system. Growth hormone supports tissue repair. The immune system reorganizes its defenses. Cytokine production follows circadian rhythms. Memory consolidation occurs. Hormones involved in hunger, metabolism, stress regulation, and glucose control are carefully coordinated.¹
In many ways, sleep is when yesterday’s damage becomes tomorrow’s resilience. Research consistently demonstrates that even modest sleep deprivation can impair natural killer cell activity, alter inflammatory signaling, reduce vaccine responsiveness, and increase susceptibility to respiratory infections.²,³ That doesn’t mean every poor night’s sleep will make you sick. It means the body slowly loses reserve when adequate recovery becomes the exception rather than the rule.
One of the most practical recommendations I give patients has nothing to do with sleeping longer. It has to do with sleeping more consistently. Our circadian rhythm thrives on predictability. Going to bed and waking at approximately the same time each day provides the brain with reliable timing cues that influence hormone production, body temperature, digestion, immune regulation, and countless other physiological processes. Consistency often matters just as much as duration.
For patients who struggle with sleep, I encourage them to begin by focusing on the hour before bed rather than the eight hours after it. Dim the lights. Reduce screen exposure. Keep the bedroom cool. Avoid large meals and alcohol close to bedtime when possible. These simple habits may seem unrelated to immune health, yet they directly influence one of the body’s most important opportunities for repair. Think of it this way, you spend your entire day giving to others – your kids, your spouse, your work, your “followers” aka social media – let that last hour be your gift to yourself. Don’t give any more to the world – take something for yourself that is nourishing, joyful, and restful.
Muscle Is an Immune Organ
One of the most exciting developments in medicine over the past decade has been the growing appreciation for skeletal muscle. For years we viewed muscle primarily as something that allowed us to lift groceries, climb stairs, or improve athletic performance. We now know that muscle is also an endocrine organ.
As muscles contract during exercise, they release signaling molecules called myokines that communicate with the immune system, adipose tissue, blood vessels, liver, and brain. These compounds influence inflammation, insulin sensitivity, tissue repair, and metabolic health in ways we are only beginning to fully understand.⁴
This helps explain why resistance training consistently appears in studies examining healthy aging, immune resilience, metabolic health, and longevity. The benefits extend far beyond strength.
Maintaining muscle also becomes increasingly important during illness.When the body faces infection, surgery, or injury, protein requirements increase as immune cells multiply, damaged tissues are repaired, and inflammatory proteins are produced. Muscle serves as an important reserve of amino acids that can be mobilized when demands suddenly increase. Individuals with greater muscle mass often tolerate these physiological challenges more effectively because they begin with a larger reserve.
I sometimes tell patients that strength training is not simply preparing them for their next workout. It is preparing them for the next unexpected challenge life brings. That challenge may be a virus. It may be surgery. It may be caring for a loved one. It may simply be getting older.
The stronger and more metabolically healthy your body becomes today, the more options it has tomorrow. Muscle is literally your built-in life insurance policy.
Food Is More Than Fuel
Patients often ask me which foods are “good for the immune system.” I understand why. Nutrition advice has become increasingly focused on identifying individual foods that promise extraordinary health benefits. Blueberries; turmeric; garlic, ginger, fermented foods…
While each of these foods has interesting research behind it, I think we sometimes lose sight of the bigger picture. The immune system doesn’t eat blueberries. It builds cells. Every white blood cell, every antibody, every enzyme involved in detoxification, every antioxidant system, every hormone receptor, every cell membrane is constructed from nutrients supplied by our diet.
Protein provides the amino acids needed to build immune cells and repair tissues. Essential fatty acids become part of cellular membranes and contribute to inflammatory regulation. Vitamins and minerals function as cofactors for thousands of biochemical reactions occurring every second throughout the body. Fiber nourishes beneficial bacteria within the gut, which in turn produce short-chain fatty acids that influence immune development, intestinal barrier integrity, and inflammatory balance.⁵
When viewed through that lens, nutrition becomes much more than calorie intake. It becomes construction material. If you’ve ever read a nutrition label outside the US, you might have noticed that calories are labeled as “Energy” – I just love that! The body cannot build resilient physiology without reliable access to quality building materials, or energy. This is one reason I rarely encourage patients to chase nutritional perfection. Instead, I encourage consistency.
Aim to include adequate protein with each meal. Most people can benefit from more than they think.
Fill half your plate with colorful vegetables whenever practical. I like aiming for 10 colors daily (and different shades of the same color count as different colors).
Choose healthy fats regularly. Examples include fish, avocados, olives, coconut, nuts and seeds.
Include fiber-rich foods daily. I tell patients to aim for 35g daily to support cardiovascular, blood sugar, and bowel health.
Eat enough to support your activity level rather than chronically under-fueling!!! I can’t say this enough. If you underfuel and over work-out, you will mess up your metabolism until it thinks your starving and will hold on to everything. This is hard to fix. Work with your body, not against it.
Small improvements maintained over years almost always outperform dramatic dietary overhauls that last only a few weeks.
Stress Was Never Meant to Be Continuous
The human stress response is brilliant. If we suddenly encounter danger, our bodies rapidly increase heart rate, blood pressure, blood sugar, and alertness while temporarily redirecting energy away from digestion, reproduction, and long-term repair. That response helped our ancestors survive.
The challenge is that the body cannot easily distinguish between escaping a predator and answering emails late into the evening while worrying about finances, deadlines, aging parents, or raising young children. The physiology is remarkably similar.
Stress itself is not the problem. In fact, stress can be a real ally. Recovery is often the problem. Healthy physiology expects periods of activation to be followed by periods of restoration. When those restorative periods become increasingly rare, the nervous system, endocrine system, and immune system all begin adapting to a world they perceive as continuously demanding.
This is why I often encourage patients to schedule recovery with the same intentionality they schedule work meetings. Recovery is not laziness. Recovery is biology. It is the period during which the body processes, repairs, recalibrates, and prepares for whatever challenge comes next. The remarkable thing about all four of these foundations is that they rarely work independently.
Sleep influences food choices. Nutrition influences exercise recovery. Exercise improves sleep quality. Stress alters digestion. Digestion influences immune function.
Each system strengthens the others. That is exactly how physiology was designed.
Where Supplements Fit Into the Conversation
By this point, you may be wondering if I’ve forgotten about supplements altogether. I haven’t.
In fact, I recommend nutritional supplements every single day in clinical practice. They can be incredibly valuable tools, and there are many situations in which they are not simply helpful but necessary. Correcting nutrient deficiencies, supporting mitochondrial function, improving digestive capacity, modulating immune activity, restoring the microbiome after antibiotic use, or helping resolve inflammation are all areas where thoughtfully selected supplements can play an important role.
The key word, however, is support.
Supplements work best when they are supporting healthy physiology rather than trying to replace it.
Imagine trying to grow a healthy garden. Fertilizer can improve the quality of the soil, but it cannot compensate for the complete absence of sunlight or water. Likewise, watering plants more frequently cannot overcome soil that contains no nutrients. Healthy growth depends on multiple conditions working together. The human body is no different.
Supplements are often the fertilizer. They provide additional nutrients, influence specific biochemical pathways, or help address deficiencies identified through a patient’s history or laboratory testing. Those interventions can be incredibly effective, but they are most effective when the body is already receiving the fundamental inputs it requires to function well.
This is one of the reasons individualized care matters so much. Two patients may present with similar symptoms while needing completely different treatment plans. One person may truly benefit from additional immune support because of an underlying nutrient deficiency or increased physiological demand. Another may be sleeping four hours each night, skipping meals, and living in a state of chronic stress. In that situation, prescribing more supplements without addressing the underlying foundation often leads to frustration for both the patient and the practitioner.
As doctors, our responsibility is to identify where support is most needed. Sometimes that support comes in the form of a carefully selected botanical medicine. Sometimes it is nutritional. Sometimes it is pharmaceutical. And sometimes the most impactful recommendation is helping someone finally prioritize sleep, protein intake, resistance training, or recovery. It’s not sexy. It’s not fashionable. But it’s truth.
Good medicine is not about choosing one approach over another. It is about understanding what the body needs most at that particular moment.
Preparing for the Stressors You Can Predict
One of the reasons I enjoy discussing immune resilience during the summer months is because this is one of the few times each year when we can prepare before many predictable stressors arrive.
As August approaches, children return to school, routines become busier, travel often continues, and respiratory viruses begin circulating more frequently. While we cannot eliminate every exposure, we can often improve how prepared our bodies are to respond. This is where proactive care becomes so valuable.
Rather than waiting until the first signs of illness appear, this is an excellent time to evaluate whether the foundations we have discussed throughout this article are truly in place. Are you consistently sleeping enough to recover? Is your nutrition providing adequate protein, healthy fats, fiber, vitamins, and minerals? Are you maintaining muscle through regular resistance training? Have you created opportunities for stress recovery rather than simply moving from one demanding season directly into the next?
Once those foundations are in place, targeted supplementation often becomes far more effective.
Depending on the individual, this may include nutrients such as vitamin D, zinc, vitamin C, omega-3 fatty acids, probiotics, medicinal mushrooms, or other evidence-informed therapies selected to address specific needs rather than following a one-size-fits-all protocol. The goal is not to take every supplement available. The goal is to provide the body with the additional support it needs while respecting that every patient arrives with a different medical history, lifestyle, and set of physiological demands.⁶
This is also one of the reasons I encourage patients to avoid comparing treatment plans with friends or family members. A protocol that works beautifully for one individual may be entirely inappropriate for another because the underlying reasons for their symptoms are different. Treating the person rather than the diagnosis remains one of the cornerstones of naturopathic medicine.

The Foundations Make Every Treatment Better
One of the most rewarding parts of practicing medicine is watching patients reach the point where their body begins responding differently. Sometimes they tell me they have more energy than they’ve had in years, or they notice they recover more quickly after exercise. Sometimes they stop catching every illness that circulates through the office or they notice their laboratory markers begin improving.
Those moments are incredibly gratifying, but they rarely happen because of one supplement, one appointment, or one dramatic intervention. More often, they occur because dozens of small decisions gradually change the environment in which the body operates. The body finally has enough protein to rebuild tissue; it finally has enough sleep to regulate inflammation appropriately, blood sugar becomes more stable; muscle mass improves; stress becomes more manageable; digestion becomes more efficient and the gut microbiome begins shifting toward greater diversity.
None of those changes happens overnight, yet together they fundamentally alter the physiology we are trying to influence. This is why I believe the foundations of health deserve far more attention than they often receive. They are not the “simple” part of medicine. They are the part that allows every other intervention to work better.
Your Doctor Can’t Heal You Alone
One of the greatest privileges of practicing medicine is watching patients realize they have far more influence over their health than they were initially led to believe. That realization is incredibly hopeful.
It means your future is not determined solely by your genetics, your diagnosis, or the laboratory values printed on a piece of paper. While those factors absolutely matter, they exist within a body that is constantly adapting to the choices, environments, and experiences it encounters every single day.
As doctors, we have an important role to play. We can listen carefully, investigate thoroughly, interpret complex information, identify patterns, recommend treatments, and help remove obstacles to healing. We can educate, encourage, and adjust the plan as new information becomes available.
But eventually every treatment plan reaches the same place. It leaves the office. That is not where healing ends. In many ways, it is where healing truly begins.
The meals you prepare, the sleep you protect, the walks you take, the workouts you complete, the supplements you remember to take, the moments you choose recovery over constant productivity, and the habits you repeat day after day ultimately shape the physiology we are both working so hard to improve.
That responsibility can feel overwhelming at first, but I prefer to see it as empowering. It means you are not simply waiting for your body to heal. You are participating in the process.
One of the most consistent observations I’ve made after years in practice is that extraordinary health rarely comes from extraordinary interventions. It grows from ordinary decisions repeated with extraordinary consistency. Health is not built in a single appointment. It is built in the thousands of moments between appointments.

As you’ve probably noticed throughout this month’s series, the gut has quietly appeared in nearly every conversation we’ve had. That is not by accident. If there is one idea I hope you carry with you from this article, it is this:
The body is remarkably intelligent. It is constantly trying to adapt, repair, protect, and restore balance using the resources available to it.
Our role as doctors is to help identify what those resources should be.
Your role is to provide them consistently enough that your body has the opportunity to do what it was designed to do all along.You cannot out-supplement your foundations. But when those foundations become strong, everything else has a much greater opportunity to succeed.
